In-Home Service Medicaid

      Waiver Program / Personal Care Program

      

 

     An alternative to living in a nursing home, the In-Home Service program allows qualified individuals  and disabled residents to receive care while they remain at or return home.  The program provides a range of services comparable to those individuals would receive in a nursing facility -- personal hygiene assistance, nutrition, arrangement for medical and nursing care, supervising the administration of medication and environmental maintenance.  Individuals 18 years or older must be eligible for Medicaid under special guidelines and deemed appropriate for in-home services by our case management agency.  Each case is ordered by a physician, coordinated by the agency.  Our nurses supervise the client homemaker service.

     The Personal Care program provides services to people age 18 and over who are eligible for Medicaid, and determined by a physician to need services because of physical or mental impairment. Services include activities related to personal hygiene, dressing, feeding, nutrition, environmental support functions, and health-related tasks.
